Pre-Visit Instructions

Help us reduce wait times and avoid misunderstandings with regards to coverage.

Prior to your appointment, kindly send the following via text message to us at (404) 205 - 5669 or via email to meganli@summerhilleyecare.com:

  • Pictures of front & back of medical insurance card

  • Pictures of front & back of vision insurance card (skip this if your vision benefits are through VSP, Cigna, or MetLife)

  • Legal name (and preferred name, if different)

  • Date of birth

  • Last four digits of SSN

If you are not the policyholder, please additionally provide the same details for the policyholder.

If you have coverage through a Google employee plan, please provide the Google Employee # of the policyholder.

All our services and products are HSA / FSA eligible.

Regardless of network status, we can always see clients on a private pay basis, and are happy to assist with any documentation needed to submit a claim for reimbursement.

[NOTE] Vision benefits for these plans are contracted to and managed by a separate, third-party vision provider network, most commonly VSP, EyeMed. Please refer to your plan details or contact your plan administrator directly to determine the specific network affiliation your plan utilizes.

Our staff is fully vaccinated.

Masks are optional within the retail gallery, though our clinicians follow stricter guidelines, which you are welcome to as well.

We observe robust protocols to ensure the safety and comfort of both clients and staff, including:

  • Limited Studio capacity

  • Appropriately spaced seating

  • Routine hand sanitization and hygiene standards

  • Regular cleaning of contact surfaces between clients
